ConnectMyHealth pulls data, in real-time, from participating hospitals. This webpage tells you which hospitals contribute data, as well as the types of data that is contributed. Use the interactive map below to see if health records from your hospital are available in ConnectMyHealth, or view the hospital organizations by name in the list below. Additional information about the types of health records that can be viewed in ConnectMyHealth is also available on this page.

Health records from your Primary Care Provider (i.e. Family Physician) are not viewable in ConnectMyHealth.
The following information is restricted, and not available in ConnectMyHealth, for all users:
- HLA tissue typing
- Forensic reports
- Autopsy
- Toxicology (relating to autopsy)
