About Registering for Your ConnectMyHealth Account

Before registering, you are encouraged to visit other sections of this website to learn more about ConnectMyHealth, and you can also review the ConnectMyHealth User Agreement to become familiar with the terms of use you'll be prompted to accept when you log into the portal for the first time, and annually thereafter. 

IMPORTANT: 

• ConnectMyHealth can be used to view select health records from participating hospitals located in south west Ontario – click here to see the participating hospitals, and what types of data may be available.   
• Be sure to use the appropriate registration form that will become available to you once you click “Register Now” below. The difference between the two registration forms is whether you want to be able to view your own health records (in addition to those of someone else (i.e., records shared with you by another ConnectMyHealth user, or as a Proxy – if applicable to you), or only the records of someone else (records shared with you by another ConnectMyHealth user, or as a Proxy). 
• You cannot submit information about or on behalf of anyone else. The information you provide in your registration form must be complete, accurate and true, and must relate only to you.
