About Registering for Your ConnectMyHealth Account

Before registering, you are encouraged to visit other sections of this website to learn more about ConnectMyHealth, and you can also review the ConnectMyHealth User Agreement to become familiar with the terms of use you'll be prompted to accept when you log into the portal for the first time, and annually thereafter.

IMPORTANT:

ConnectMyHealth can only be used to view select health records from participating hospitals located in south west Ontario – click here to see the participating hospitals, and what types of data may be available.

You are reminded that you can only request access to your own personal health information through a ConnectMyHealth account, and that you cannot submit information about or on behalf of anyone else. The information you provide in your registration form must be complete, accurate and true, and must relate only to you.

Begin Your Registration

If you are 16 years of age or older, have a valid Ontario Health Card, and a unique email address (not shared with anyone else), you can register for a ConnectMyHealth account.

To help ensure you register successfully, you can watch this short video before you begin.

Once your account request has been successfully processed, please note you will need to set up Multi-Factor Authentication using SMS Text, or by downloading, at no cost, an authenticator app (e.g., Google Authenticator, Twilio Authy, FreeOTP) as part of the account activation process. This requirement supports your Two-Step (Multi-Factor) Authentication during each login to ConnectMyHealth for added security.
