Who Can Register for ConnectMyHealth?

To register for a ConnectMyHealth account, you must:

· Be 16 years of age or older 

· Have a valid (not expired) Ontario Health Card

· Have a unique (not shared with anyone else) email address

· Have a secondary piece of identification (see List of Acceptable Identification Documents) 

ConnectMyHealth can be used by individuals to view their health records from participating hospitals in Ontario Health's West Region. However, ConnectMyHealth also allows users to “share their records” with other individual(s) as they see fit, even if the person they are sharing with doesn’t have their own health records at participating hospitals but otherwise meet the criteria listed above. When registering for your account, use the appropriate registration form, based on whether you yourself have received care at a participating hospital(s) or not.

Parents, Legal Guardians and Legal Representatives of individuals who are unable to access, manage and control their own ConnectMyHealth account will be able to register for Proxy Access; information will be made available on this website about the Proxy Access Request process. Until then, Parents, Legal Guardians and Legal Representatives who wish to access another individual's personal health information may continue to do so via other means, including contacting the healthcare facility that created the health record.
