A “Proxy” is a person who, under certain conditions as explained below, can be authorized to access ConnectMyHealth in the place of a Patient. The Proxy must first request Proxy access, and if granted, will be able to access and use ConnectMyHealth in the same way that the Patient would otherwise have been able to.
Please read the following information carefully to understand how Proxy Access works.
1. Patient Age: Proxy access is currently only available when the Patient is at least 16 years old.
2. Proxy’s Account Requirement: To request Proxy access, you must already have your own ConnectMyHealth account; if you are seeking Proxy access but don’t yet have your own ConnectMyHealth account, register for one by visiting the Registration page on this site. The Patient themselves does not need their own ConnectMyHealth account.
3. Legal Representative Types: Based on the Patient’s current age, the following are the allowable applicant types. Excluding “Parent”, specific documentation supporting the type of legal representative applicant you identify as, will be required when submitting your Proxy Access Request Form:

Health Care Consent Act
4. Patient Capacity: Proxy access is currently only permitted when the Patient is not capable of accessing, managing and controlling their own ConnectMyHealth account. Proxies will be required to upload proof of patient incapacity (Form 33 - Notice to Patient under Subsection 59(1) of the Mental Health Act or Capacity Assessment).
5. Review & Approval Process: Your Proxy Access Request submission will be reviewed within 10 business days, and you will receive a decision by email. If approved, the email you receive will include instructions on how to access the Patient’s health record for the first time using your own ConnectMyHealth account. The ConnectMyHealth Program Office reserves the right to request further validation or clarification regarding submitted documentation.
6. Annual Attestation Process: All Proxy access includes a mandatory annual attestation process to confirm both the Proxy’s ongoing legal authority to view the Patient’s health records, and the Patient’s ongoing incapacity. Proxies will receive instructions by email when an attestation is required, and a maximum of 60 days is allowed to complete the attestation. Failure to complete the attestation will result in your Proxy access to the Patient’s health records being automatically revoked.
7. Record Sharing: Proxies will have the ability to ‘share the Patient’s records’ with another CMH user(s), such as the Patient’s family or caregiver, given the Proxy is in effect using the portal as the Patient otherwise could.
8. Patients with an Existing ConnectMyHealth Account: If you are requesting Proxy access for a Patient who already has a ConnectMyHealth account, their account will be automatically disabled once your Proxy Access is granted.
To submit your Proxy Access Request Form, log into your ConnectMyHealth account, click on “Resources”, then “Proxy Access Request Forms”.
